A detail-focused Care Plan Assessor is sought to support the delivery of high-quality domestic support services for our aged care partner. The role is ideal for individuals already in the aged care sector and who have an interest in exploring opportunities to apply their knowledge of client needs to create clear, practical, and person-centred care plans.
Working closely with care coordinators and support teams, you will:
- Write tailored care plans focused on domestic cleaning services to support at home clients (cleaning, laundry, meal preparation, and household support), in line with funding guidelines.
- Interpret assessment information and translate it into actionable service plans
- Ensure care plans reflect client preferences, routines, and cultural considerations
- Maintain accurate and compliant documentation
Applicants will have experience in aged care, home care, or community services, as well as understanding of domestic support services in a home care setting. A relevant qualification in Community Services, Aged Care, or similar is desirable.
